Program Providers

Requirements:  This membership level is available to any organization/program that:

• Agrees to support the USC’s mission and be subject to the USC’s Articles and Bylaws
• Agrees to accept the USC’s polices and procedures as a condition of membership, including the non-discrimination and standards statement and others as the Collaborative may properly adopt in the future
• Completes a membership form
• Pays an annual membership fee of $100
• Is a 501(c)(3) designated organization
• Has a primary focus of serving youth in underserved communities
• Has a youth development program component
• Has a soccer program component
